If I have to say no unconditionally, I don't feel very uncomfortable. Because there is a clear reason. However, I feel uncomfortable when I have to say no with my simple mind. Because the reason is only one of my heart, it reminds me of the idea that the other person will be disadvantaged because of me. That proposition is the cause of fear of rejection. And accordingly, the expected reaction of the opponent also plays a role. If he or she shows a reaction that is not positive and sad or in trouble, I also feel down. So there are times when I can't help but refuse the other person's request regardless of my intention.

I agree that rejection can sometimes be just as difficult/painful as saying no to anything. Positively, though, it also suggests redirecting to greater chances ^__^.
